Access Funding
NDIS (Plan or Self Managed)
Active Wise has a lot of experience of working with plan and self managed NDIS participants across all disciplines. Once you’ve confirmed what service you require we will draw up a service agreement outlining our costs upfront.
Home Care Package (HCP)
If you are on a Federal Government HCP you may have funds available to cover the cost of your allied health services. We see many self and plan managed HCP holders and work closely with yourself and/or your client Advisor to help you achieve your goals.
Pay Privately
We see many clients that pay privately for this services. If you have private health insurance you might be eligible for a rebate. Give us a ring to discuss.
Chronic Disease Management (CDM)
Clients with chronic medical conditions may be eligible for a CDM (Formerly Enhanced Primary Care or EPC) plan. A CDM plan can help offset the cost of our services. Give us a ring or talk with your GP if you think you might be eligible.
DVA
If you are a DVA Gold Card holder you might be eligible for your services to be funded this way. If you are eligible you will need to supply us with a referral from your GP. Give us a ring to discuss for further clarification.
